1. MISSION AND OBJECTIVES
The
mission is to advance the understanding of the epidemiology of CVD in developing countries and to
help develop timely and locally sound responses
-
Main issues include the strengthening of systems for collecting epidemiological data necessary for action, the application of available knowledge whenever relevant, the development of alternate or new strategies whenever needed, and the evaluation of processes and results of interventions.
-
Emphasis is given to primary prevention -in order to curb the mounting epidemic of CVD- and to affordable and cost effective interventions -in view of limited resources.
Main
objectives include the following:
-
Description and analysis of the epidemiological transition in developing countries and implications for prevention and control of cardiovascular disease (CVD).
-
Collection of data on main risk factors of CVD in populations experiencing epidemiological transition (e.g. tobacco use, lifestyles, high blood pressure, lipid disorders, diabetes, metabolic syndrome) and development of sustainable systems of surveillance in developing countries.
-
Analysis of determinants of CVD in developing countries, e.g. conventional and other risk factors, socio-economic factors, perinatal data, genetic factors.
-
Development and analysis of strategies for the prevention and control of CVD in populations and high-risk individuals, including:
-
development of sustainable surveillance systems,
-
advocacy targeting the general public, health professionals and policy makers, including the development of relevant materials and participation in awareness campaigns, and interaction with key players,
-
standard settings, e.g. development of locally sound, evidence-based, and relevant guidelines for the management of high blood pressure or diabetes,
-
health policy development, e.g. tobacco control legislation, nutrition policy.
-
Capacity building related to activities described above.
-
Sharing expertise with governmental and non-governmental organizations in the area of CVD epidemiology and prevention in populations in epidemiological transition.
2. SUMMARY OF RESULTS OBTAINED
Activities of CVD research and prevention have concentrated mainly in Seychelles (since 1989-ongoing) and Tanzania (since 1998). Concurrently, expertise was shared
with organizations pursuing similar goals.
Main achievements, and related publications, include:
Epidemiological research

Population based surveys of CVD risk factors in Seychelles (1989, 1994, 2004) and in Tanzania (1999).

Distribution of risk factors in children in Seychelles (surveillance system established in 1998-ongoing).
-
Distribution and determinants of subclinical outcomes of CVD such as carotid/femoral IMT and atherosclerosis (Seychelles).
-
Genetic determinants of hypertension (Seychelles).
-
Role of birth data on risk factor levels in children (Seychelles).
- Longitudinal analysis of the relation between risk factors and CVD outcomes (Seychelles).
- Compliance to antihypertensive treatment and determinants (Seychelles).
- Cost effectiveness of smoking cessation (Seychelles).
- Impact of health education on lifestyles and risk factor levels (Tanzania).
- Health seeking behaviors related to high blood pressure (Tanzania).
- Various methodological issues related to blood pressure (measurement, assessment of prevalence in epidemiological studies) (Seychelles, Tanzania)
- Methodological support to research in other areas (hepatitis, HTLV, leptospirosis, etc) (Seychelles)
Policy development

Initiation of and participation to national committee for tobacco control (Seychelles), international negotiations of the Framework Convention of Tobacco Control (as delegate of Seychelles in all INBs and spokesperson for AFRO in some sessions), and contribution to drafting tobacco legislation (Seychelles)

Participation in the development of nutrition policy (Seychelles)
Standard setting

Elaboration of national guidelines for diagnosis and management of high blood pressure, diabetes, and blood lipid disorders (Seychelles).
Training

Training through formal lectures and workshop related to lifestyles, nutrition and strategies for the prevention and control of CVD and related risk factors (Seychelles, Tanzania)
- Organization of various seminars and conferences to disseminate findings of local surveys (Seychelles, Tanzania)
- Training of health professionals on evidence-based practices for the diagnosis, management and prevention of CVD conditions
- Participation as faculty to seminars on CVD epidemiology (WHF10-Day Seminar, IUHPE-CDC seminar)
- Development and conduct of a 2-week course for the development of prevention programs in several developing countries (given on the location of requesting organizations)
Advocacy, awareness at local level
- Active involvement in high-profile health promotion activities (e.g. World Diabetes Day, World No Tobacco Day, World Heart Day), some of them received awards (WHO World No Tobacco Day award) (Seychelles)
- Active participation in awareness campaigns, including development of educational materials and participation in numerous programs on the local media (radio, TV) (Seychelles)
Capacity building (other than above)
- Development of functional structures within ministry of health (e.g. Unit for Prevention and Control of CVD), including relevant training of local personnel (Seychelles, Tanzania)
- Development and maintenance of knowledge databases of local study findings (Seychelles and Tanzania)

- Participation as experts in various CVD-related consultations or working groups, e.g. WHO, World Bank, IC Health, Global Forum for Health Research, World Heart Federation.

Appendix 1. CVD research and prevention program in the Seychelles
CVD research and prevention program in the Seychelles islands (Indian Ocean)
A CVD research and prevention program has been initiated since 1989 as a joint project between IUMSP and the Ministry of Health of the Republic of Seychelles. The program aims at 1) describing and analyzing the cardiovascular disease situation in a country experiencing rapid health transition and 2) developing corresponding prevention and control strategies. Population-based surveys in 1989 and 1994 showed high levels of several main risk factors, particularly hypertension as well as some CVD outcomes (peripheral atherosclerosis as measured by high performance ultrasonography). A Unit for Prevention and Control of Cardiovascular Disease (UPCCD) was set up in 1992 in the Ministry of Health of Seychelles and several public health measures have been developed and implemented. This includes media campaigns to rise public awareness, the development of various public health policy instruments, the integration of health education on non-communicable diseases within the regular curriculum in schools, hypertension and diabetes screening programs, ‘heart health clubs’ to improve risk factors control in high risk individuals attending health centers, surveillance systems such as a health-center based national register of hypertension and diabetes among adults and a screening program of hypertension and overweight in all primary and secondary schools. Ongoing studies include a cohort study of risk factors, surveillance of risk factors integrated in school programs, relationship of risk factors in children with birth data and a study of genetic determinants of hypertension. A study of the prevalence of smoking habits and associated factors was conducted in 2002 (Global Youth Tobacco Survey) in collaboration with CDC and WHO. Further policy development in 2002 included the formulation of a national program for tobacco control and ad hoc legislation through a National Committee of Tobacco Control and the formulation of guidelines for the diagnosis and clinical management of hypertension, diabetes and dyslipidemias.
This research and prevention program is described in some detail in several published articles, for example in CVD Prevention 1999;2:261-272;108-112; Cardio Update (New Delhi) 1996;July:10-22; Soz Praventivmed 1995;40:35-43; Wrld Hlth Statist Q 1993; 46(No. 2); 108-112; and Soz Praeventivmed 1991; 36(Suppl. 1): S30-3, Insight Health (May 2001). More specific issues and results have been reported in ~20 other publications (abstracts and full texts are available on the web site of the GCT, address appearing on the cover page of this report).
The UPCCD in the Ministry of Health of the Republic of Seychelles is staffed with 5 local health workers and complemented with consultants from IUMSP (one on a long term basis attachment, PB). Projects have benefited from funding and resources from the Swiss National Science Foundation, various divisions and departments of the Faculty of Medicine of the University of Lausanne (Switzerland), the Service des Hospices Cantonaux (Vaud), the Swiss Canton of Jura, and the Swiss Agency for Development and Cooperation.

Appendix 2. Hypertension in a population in an early stage of epidemiological transition
It aims at examining the epidemiology of hypertension and other risk factors; knowledge, attitudes and practices on hypertension, and utilization of health services for hypertension in Dar es Salaam, Tanzania. This research program has been initiated in collaboration with the Tanzanian Health Authorities since 1998 and with the support of the Swiss National Foundation for Science.The study takes place Dar es Salaam, Tanzania. The study examines the prevalence hypertension and its determinants, the strategies for efficient diagnosis of hypertension, the patterns of utilization of health services by hypertensive patients and the effect of health education as a mean of blood pressure control in hypertensive patients. These data will help inform strategies to prevent, detect, and treat hypertension in low-resource settings.

This research program has been initiated with the Tanzanian Health Authorities since 1998 and with the support of the Swiss National Foundation for Science. As cardiovascular disease is rapidly emerging in developing countries, this research project investigates epidemiological and public health characteristics of hypertension in an urban population in early epidemiologic transition (Dar es Salaam, Tanzania). Research includes prevalence and determinants of hypertension, strategies for efficient diagnosis of hypertension, and utilization of health services by hypertensive patients. After main components of the study were organized and piloted in 1998, 17 local clinical officers collected in 1999 data on blood pressure and related factors in nearly 10,000 adults of five branches of the capital Dar es Salaam. Preliminary results show higher than expected prevalence of hypertension and selected risk factors for hypertension have been identified. Preliminary results provide clues for optimal strategies for efficient hypertension diagnosis (e.g. based on 3 readings at each of two visits). A follow-up part of the research (which was prepared and piloted in the last quarter of 1999) will investigate in 2000-2001 how the newly hypertensive persons (as identified in the survey in 1999) have utilized health services during the one-year interval following hypertension diagnosis. When analysis is finalized, these data will help inform strategies to prevent, detect, and treat hypertension in low-resource settings.
Appendix 3. Two-week workshop ‘Starting a CVD prevention program with limited resources’
The workshop aims at teaching and practicing the main elements needed to initiate a sustainable program of prevention and control of cardiovascular diseases (CVD) and some other related non-communicable diseases (NCD) in regions with limited resources. It combines teaching sessions on the recent scientific knowledge and an extensive practice of analysis and program development tools. In general, the course is organized in the requesting region/country from where participants come from.
The objectives of the workshop are to: 1)·To raise awareness of the participants on the emerging epidemic of NCD/CVD in low and middle income countries; 2) ·To enable the participants to identify and analyze the current CVD situation in their respective countries; 3) ·To equip the participants with the knowledge and skills to initiate a prevention and control program of CVD; 4) ·To build a network of health professionals in low and middle income countries in the field of prevention and control of CVD.
Location.The workshop generally takes place in the region/country where the participants come from. This avoids traveling expenses to the participants and it helps ensuring that the content of the course (epidemiologic assessment, development of a prevention program) is adapted to local needs. The workshop is held in a secluded place (e.g. a hotel away from a main center) that offers accommodation facilities for the sojourn of all participants during the entire workshop’s period. The workshop can be given on request of organizations/individuals from low or middle income countries and date and location (in the host country) are then chosen accordingly.
Lectures aim at delivering the minimal information needed to develop a program in terms of relevance, advocacy, goal setting, and actual organization. All lectures are completed with ‘check lists’ and handouts as resources for the participants. A quick individual assessment is performed at the end of the workshop as an indication of knowledge integration.
Practice aims at training participants with tools needed at the various steps of program development: epidemiologic relevance, situation analysis, advocacy, identification of resources, design, implementation, and evaluation. Participants will conduct, within small groups, a rapid epidemiologic assessment in the field of CVD/NCD (during the first week) and draft the elements of a hypothetical comprehensive CVD prevention and control program (during the second week) adapted to the situation of the participants’ country/region. The main principle of the training sessions is ‘learning by doing’. Along this training, participants will use computers and word-processor, spreadsheet, and statistical software. No previous computer literacy is requested and computers will be made available (at least one computer per group). Participants will present their work (epidemiologic assessment, program draft) in plenary for critical appraisal and learning.
Participants are health professionals from countries with limited resources, preferably with some experience in public health and/or health research (at least two years of working experience). Fluency in English is requested for course in English. Alternatively the course can be given in French (in which case fluency in French is requested). The number of participants is limited to approximately 15-20.
Facilitators include Dr. Pascal Bovet, MPH and Dr. Jean-Pierre Gervasoni, MPH from the Group of Cardiovascular Disease and Epidemiological Transition (GCT) of the IUMSP, Lausanne. The IUMSP is a collaborating center of the World Health Organization since 1994 for population-based cardiovascular research and prevention in populations in health transition. The faculty of the workshop includes generally also one or two other facilitators either from IUMSP or from other Universities.
The workshop has been organized in Bagamoyo, Tanzania, 9-21 Apr 2000 (participants from Ministry of Health, Tanzania) and in Kulhudhuffushi, Maldives, 30 Sep-14 Oct 2000 (participants from Ministry of Health of Maldives);
