The role of evidence synthesis in directing future research has been prominent in opinion articles but empirical evidence has shown that the majority of randomized controlled trials do not consider existing meta-analyses to inform their design. I will present a framework, termed conditional trial design aiming to estimate efficacy or safety of interventions based on network meta-analysis.  The framework is relevant only to cases where the motivating research question aims to inform post-marketing decisions, such as guideline development and reimbursement. I will show how conditional trial can optimize research investment by detecting evidence gaps and can indicate an optimal new study design that will update the existing evidence to best serve public health decision making.
